I'm writing this because of a womans problems she's had after stress dosing. She doubled her Medrol from 6 to 12 mg for 4 days. The 5th day she went straight back to 6 mg. She promptly seemed to go hyper thyroid. Had the symptoms, pulse and bp went up, temp stayed the same though.
She went to the emergency room within 24 hours of lowering the Medrol. They told her to get off the Armour immediately and put her on beta blockers which doesn't seem to be helping. I told her to raise the Medrol to 10 mg and that doesn't seem to be helping either, so told her to try lowering her Armour (3 3/4 grains per day) at 1pm by 1/2 grain to see if it really is the Armour causing her problems. If she then gets better, then we'll go from there to figure out what to do next.
I've said this many times, but I don't think is enough, so it bears repeating. If stress dosing (doubling) more than 2 days, then taper down to your replacement dose gradually. Being at 12 mg of Medrol like she was for 4 days, I would have recommended she try lowering to 10 mg on the 5th day and lowering by 2 mg every 3rd day after that.
She was doing extremely well self treating (probably the best I had ever seen) and now it's like a monkey wrench has been thrown into the works. I hope I can help her recover and get back to where she was before she stressed dose.
