Hi, I've got a docs appt on Wednesday and wanted to check the following beforehand.
I recall reading that a urinary cortisol test is of no validity when taking HC.
Is this because the cortisol test will not give an accurate picture of what the adrenals are producing, or is it because the HC inaccurately skews the level of HC in the urine? My test was well over range so I'm wanting confirmation of the meaning in case my doc wants me to lower my dosage (40mg) of HC.

Hi Josh Some doctors do use HC testing when the patient is on cortisol and some don't think it's valid. It's up to the doctor. It might be more that the ACTH, AM cortisol and the stim testing is not relevant when on HC. Did Dr L recommend that you have this test done or did you request it? If you feel good on 40mg HC then you should express that. Dr L does believe in treating symptoms rather than just using tests (though she didn't want Mum over 35mg). Happy Friday! Enjoy the weekend

24 hr U Cortisol is the most 'accurate' test available to measure cortisol replacement BUT, all in all, it is totally useless. If anything, it will only give an indication that you're taking too much, at the wrong times or excreting too quickly.

Yep, she did request that I have the test. Last time when I said I was on 25mg she said I can increase up to 40mg. We'll see what she says!
