Saliva results 6-8am............ - 13 Normal (range 13-24) 11-12am.......... - 2 Depressed (range 5-10) 4-5pm ..............- 4 Normal (range 3-8) 10-12pm.......... - 7 Elevated (range 1-4) Cortisol Burden - 26 (range 23-42)
DHEA - 3 Borderline (range 3-10)
This would definitely explain my insomnia. If I'm reading this correctly, it would also help explain my lack of energy during the day, would it not?
Below are the interpretations that came with the results...they're just hard for me to understand:
Marginal HPA axis performance during the day may be associated with suboptimal hypothalamic pacing of adrenals. Adrenal support suggested.
Midnight value is elevated suggesting a lack of sensitivity to suppression at the pituitary-hypothalamic-axis. This condition is usually associated with a tendency to endogenous depression, and REM sleep disruptions. Phosphorylated serine derivatives are reported to help optimize HPA responsiveness. Clin. Trial J. 26:33, 1989.
Minimal cortisol rhythm; cortisol augmentation and anabolic support suggested.
Sex Hormone results: Estradiol............... - 21 (range 5-13) Progesterone....... - 84 (range 20-100) Free Testosterone - 20 (range 8-20)
I find the Testosterone especially interesting, as all of my blood tests have shown it on the low end of the range, and this shows it on the high end

Optimal Cortisol is TOP of range in am (yours is BOTTOM), near top at midday (yours is DEFICIENT), about midrange in latavo/early evening (yours is only20% in range) & bottom of range at Midnight (yours is elevated). This means very little Cortisol at crucial times of day when needed for energy. And too much at night (your body attempt to "catch up") when you need to sleep causing insomnia & poor quality sleep.
DHEA is optimal at top 25% to top of range. But see comments below about Sex Hormones.

Trial J. 26:33, 1989. In other words your Cortisol levels at night are too high & can cause proo quality sleep, difficulty getting to sleep, & depression. They are suggesting PS be considered to lower night time Cortisol but I think thats a mistake in your case (see your PS thread).

Certainly NOT for Estrogen, Progesterone or Testosterone.
Your DHEA saliva was BOTTOM of range. This suggestes DHEAS & TEtsosterone would be low/deficient as well. This is what you say has been the result in previous blood tests.
You need to test Sex Hormones in blood BEFORE doing any Sex Hormone HRT. What is needed is COMPLETE testing done ~ day 3 of cycle at 8am fasting 10-12 hours overnight. All the following should be done AT THE SAME TIME
•Hormonal Panel (early 8am fasting overnight) includes FSH LH Estradiol (E2) Estrone *available USA Estriol *available USA Progesterone LH/FSH Ratio
•Androgens Panel (early 8am fasting overnight) includes Testosterone SHGB FAI DHEAS Androstenedione Free Testosterone
By the way your Saliva results definitely confirm you have AI. The DHEA result plus previous low Testosterone blood results also suggest that you have Secondary AI rather than Primary AI. Meaning you look Hypopituitary for ACTH which is causing the low Cortisol. Did you have 8am Cortisol & Serum ACTH tested as yet? if so please post results. If not PLEASE get this done BEFORE starting any HC.
Also note it is recommended that HC be started BEFORE or at the same time as DHEA/Testosterone HRT.
