This is what I can post about it:
(First days of March) The daughter of my boss (and my boss) is going through a nightmare. I had never been told that she was going through such health problems until yesterday, when my boss explained to me that they were in the Hospital as she was having a Lumbar Puncture done. Preliminary results have said that she has no tumour, which is a relief, but it seems they still have to analize it for other conditions.
Today, I have been able to talk further with my boss and I have been given more information on her daughter´s health issues. According to the Neurologist that did the lumbar puncture they suspect of Tuberculosis, Lyme disease.... (It seems that they have not much idea)...
My boss has explained that since she was a child, she has suffered from weakness, tiredness and dizziness. She sometimes even has lost consciousness (syncope). Doctors have never been able to determine the cause of it.
For the surprise of doctors, nowadays they have noticed that her pupils flutter all the time (constrict and dilate)... Again, they do not have any idea of the reason.
She has been to the ER not long ago because she had 104º F of fever. Doctors were unable to know why this episode happened.
The problem now is that from December, another problem has arosen. It has happened twice I think. One day, she got up and noticed that she was seeing double. It continued for about 24 hours. She is a dancer (I know, not a good idea) and had a performance that night. In fact she got the job not long ago, I think about November last year (which means she has been having a lot of stress added + physical exercise). Of course, she had a very bad performance, as she continuosly throbbed herself against walls and her companions, unable to know where they were (as she was seeing them double).
Furthermore, I have been told that from December to now, she has lost a lot of weight and feels very weak. Even though, as she has a contract with the performance company, she is planning to return to work and start dancing on Saturday.
Everything I have been told by now seems to direct us to some adrenal insuficiency... Am I right? I am pretty worried about her wondering if having this double vision from time to time could mean that her axis (pituitary/adrenals/hypothalamus) could be seriously damaged... And that her health is going all the way down very fast.
Do you agree that this could be caused by the adrenals, pituitary, hypothalamus...? What do you think she should be tested for? What would you recommend her to do as soon as possible? Is the double vision related with the pituitary? Any idea?
Update: She has been back to work. For a few days, she still had headache (from the test) but now she seems better. But... For how long? Thank you for your help, I deeply appreciate it, Love, Tessa

Hi Tessa,
I think you are right. She needs a thorough endocrine work-up ASAP. Might also need MRI of pituitary with and without gadolinium. The visual disturbances are concerning. Any reports of enhanced sense of smell? Loss of peripheral vision?

Recommended labs to start: Morning cortisol, morning ACTH,( DHEA-S would be lovely too) Fasting glucose Aldosterone and Renin. Restrict salty foods/processed foods for 24hours prior and have her sit up for two hours prior to test. Do not lay down. May walk from house to car and from car to lab. Ferritin Vitamin B12 TSH,FT4, FT3
Depending of these results, she may need to have all pit hormones tested.
Would it be possible to test blood pressure and pulse lying down and then upon immediately standing up?

Hi Tessa,
Just wanted to add that she should definitely have blood sugars thoroughly checked out. That seeing double could be related to problems there.
Make sure that Diabetes & Hypoglycemia have been ruled out. That would mean Fasting Insulin, Fasting Glucose, hbA1C at least. Ultimately an Insulin & Glucose Tolerance Test should be done. Thats where a measured dose of Glucose is drunk after baseline Insulin & Glucose blood levels are taken. Then these are retested every hour for 2 hours at least (some do up to 4 hours)& results analysed.
Make sure Sodium & Potassium is tested with Aldo & Renin.
With the weight loss it can be low Cortisol or alternatively could be Graves (Hyperthyroid) or Hashis Hypothroidism. So she should have TPO Abs & TG Abs for Hashis tested. Also TRAbs for Graves.
