I would recommend a baseline DHEA-S level along with morning cortisol and morning ACTH level. I would wean the DHEA and be off DHEA for two weeks prior to testing to allow HPA axis to reset and get truer levels.
After testing he can try taking melatonin at night. Melatonin opposes cortisol and may help with sleep. Please see the melatonin forum for more info.
Be sure he eats a protein snack before bed. This will help prevent hypoglycemia. Low cortisol folks wake up due to hypoglycemia. His elevated cortisol at night may be due to lack of sleep, waking up startled and even sleep apnea. I don't know if these are factors.
Does he have any other previous labs(beside the saliva test)? DHEA-S,sex hormones..?
You are welcome, Laurie
