Hi there, I am on another board about thyroid. And they have different rules. There is a girl on there who is worried about her pituitary. But when you respond, you can't give other websites or there is no place to pm or anything like that. So I told her I would try to get an answer for her.
ACTH 8 am 19.70 (0-46) HGH 0.69 (0.06-5) a.m cortisol 8 am 607.82 (118.6-618) You cannot tell people on this board where to go to get more help. I don't think that's right. but that's besides the point. If you can help me help her thanks.. I know how it feels to get help. This board has helped me alot. Mamie

Acth we use Lab range up to 60 which apparently all Labs USED to use some years back. So the ACTH is low.
However the Cortisol is not low. The Lab range is similar to Aussie labs. Is this friend in Canada or UK perchance? Of course that result would be TOTAL Cortisol and not FREE Cortisol. It would depend upon the Cortisol Binding Globulin levels whether the available Free levels are adequate for needs. CBG cannot be tested in most Labs in USA & Australia. It is more commonly tested in Europe.
I assume your friend is symptomatic? What are her symptoms? What other testing has been done? Is she Hypothyroid too & is she on meds for it?
She needs to get Saliva 4 x Cortisol testing done to check on Cortisol rhythm & that equates to free rather than total Cortisol so is more diagnostic than blood Cortisol.
As far as GH is concerned obviously that result is low in range. However GH is released in pulsitile fashion (no particular rhythm & can vary from zero to top of range anytime) so is no way to tell if she has a GH deficiency from this test. Other tests such as Arginine Stim or Insulin Tolerance Tests can be used to DX GH deficiency. This is where an injection to stimulate GH is given then GH measured in repsonse to this. Litttle or insufficent response indicates a problem. I should point out that even if GH is deficient that treating AI, Sex Hormones, Thyroid can improve the GH. So best to delay any HGH HRT until that is done.
ACTH Stim is ultimate test of Adrenals ability to produce Cortisol with stimulation provided by an injection of ACTH. Your friend may need to get this done to really know what is going on. But first the Saliva test should be done & will give additional info.
If your friend has a problem it is likely she is Secondary rather than Primary going by the am Cortisol result. But really we dont have enough to say that is definitely the case. Other clues like Sodium, Potassium, Aldosterone, Renin, Androgens, Sex Hormones, LH, FSH, Prolactin, Lipids (Cholesterols), Blood Sugars (Fasting Glucose, Insulin, Hb A1C), TSH & Frees, Thyroid & Adrenal Abs will all add to the picture. We also as you know like to check Vit D, Folate, B12 as all important. Liver & Kidney tests (CMP) plus CNC with ESR are good to have too.
Can you get your friend to come here & post? We really need more info (history, labs past & recent, symptoms, meds & supps) in order to suggest anything else to help her.
