My bestfriend of 20+ years has a lump in her throat, her thyroid (only 1 side of it) has a huge lump in it only when she swallows a drink, it buldges out only on one side. So I was concerned because she was told she has bipolar disorder type 2, because one day she will be hyper and the next she will be down depressed and in bed. So I had her test her TSH, I was shocked when it came back normal, it was 0.6 (range is .450 to 4.50) But her sodium levels were high at 146 (range is 135 to 145). Her doctor said her sodium was high because she was drinking beer the night before the sodium blood test?? Her potassium was 4.7 (range is: 3.5 to 5.2). She fluxuates between being hyper and hypo? Could this be Hashi's? Or is she just fine? Her doctor told he she was depressed and could be bipolar type 2 but I don't believe that. Any ideas?

Fluctuating between hyper and hypo would be hashi's. That .6 TSH by itself would either by the hyper swing of Hashi's (more thyroid hormone production, pushes down TSH) or secondary hypo.
From your description, it's likely she's Hashi's. Bipolar is probably Hashi's in most cases.
The beer may have raised the sodium, she needs to retest with normal diet, no beer. Potassium looks good, but if her sodium is high from the beer, that could have pushed down what might have been a higher potassium. Get them both tested again.

Hi Mia,
Your friend needs TSH, both Frees, TPO & TG Abs & RT3 tested in order to evaluate full Thyroid status.
Has she taken Basal & daily av temps? Whats her BP & HR like sitting & standing?
With that lump I would recommend an Ultrasound of her Thyroid too. Does it ever bother her eating, drinking or breathing? Any voice hoarseness?
What other symptoms (Adrenal, Thyroid) does she have?

I had read that Bipolar is Hashi's misdiagnosed. That psychiatrist diagnose you as bipolar but a medical doctor as hashi's. It's very likely that getting her thyroid and adrenals in check, she may no longer be bipolar. Look at iodine too, deficiency can cause goiter and that is what her lump is. Agreed on the testing suggested by the moderators.
